Lo primero es contactar a través de la página oficial [http://fate-go.us] y buscar "Contact us", te redirigirá al correo (support@fate-go.us) para hacer tu pedido.
Explicas tu situación de como perdiste tu cuenta, todo esto tiene que ser escrito en inglés.
En un par de horas aproximado, te deberían dejar el siguiente mensaje con un formulario:
